Clean Fill Dumping

Dear Valued Customer,

Due to the new law that was passed into the Illinois State Legislature on 08/03/2010 to amend the Clean Construction Debris Regulations per SB-3721, we have implemented new changes for dumping clean fill at our facilities.  These changes apply only to incoming Dirt; they do not apply to Concrete, Asphalt or Grindings.  Please keep in mind that we are “NOT” a CCDD facility.  Mixed loads of any kind will not be accepted!  We do recycle Asphalt and Concrete at all our locations but the loads must be clean.         

For Approval to Dump Clean Fill at our CLEAN FILL ONLY facilities, please follow these procedures.

  • First and foremost you must sign a Beverly Materials LLC. Clean Fill Acceptance Agreement which can be downloaded from our website (see right side of this page) or you can call the office at (847-695-9300) and we will be glad to email one to you. This form is Required by all persons or companies prior to being able to dump at our facilities.
  • Fill out a Beverly Materials LLC Application for Incoming Uncontaminated Fill Material Form, Incomplete forms will not be accepted.  The site operator information on all the forms would be the company or persons doing the work! The billing information will be the company or persons that Beverly Materials will be sending the dump bill to, please indicate the total # of anticipated loads or cubic yards you wish to bring to us, if you misjudge the quantity you can call and increase the order at any time. Please indicate the type of truck that will be hauling the material. We also will need the Fax# and the email address for the company or persons being billed for the dumping.  These forms must be filled Out for Every Project!!
  • For Residential or Agricultural Properties  An IEPA LPC-662 Site Owner/Operator Certification form is required, It must be signed by the owner or operator of the property stating that the property has never been used for commercial or industrial purposes and is assumed to be uncontaminated material.
  • For Commercial or Industrial Properties  An IEPA LPC-663 Licensed Engineer Certification Form must be filled out and signed by a licensed engineer or a licensed geologist!  This form is required for all industrial and commercial excavation and all road work excavation.  Along with the BM application and the LPC-663 form, we also require a soil report via (an Analytical report) or a (Phase one report) done by a professional consultant, Please submit one of these reports with each LPC-663 form submitted.
  • The LPC-662 and the LPC-663 forms can be downloaded either from our website     (see right side of this page) or (http://www.epa.state.il.us/land/ccdd/index.html)   and you can get the Beverly Materials LLC Application for Incoming Uncontaminated Fill Material from Our Website, You can also contact Denny Quick at Beverly Materials (847) 695-9300 Ext. 6146 and I will be glad to fax or Email the forms to you and assist you, or answer any questions you may have.
  • Fax or Email all forms a minimum of 2 days prior to the anticipated startup date of project  to fax # (847)-841-8382 or email to dquick@plote.com, wait for either an approval letter or a rejection letter to be sent back to you within 1-2 business days before being able to dump. Beverly Materials will conduct an internal IEPA Environmental Concerns search for your project prior to accepting any material at our facility.
  • Upon Approval you will be issued an approval number and your clean fill will be accepted at our Hoffman Estates facility, every load must be accompanied by the approval number issued to that specific job.  There will be NO EXCEPTIONS for anyone, all forms must be filled out and signed, then approved, prior to being able to dump at any of our facilities!!
